The issue that I am seeking to improve is on a NICU floor by implementing an educational channel called the Get-Well Network. I currently am not working in the healthcare setting; however, I spoke to a friend of mine who works on a NICU floor about my capstone project. My friend mentioned she would love to be my preceptor for this project! She discussed with me an issue she has witnessed on the NICU floor she works on. She mentioned how she used to work on the medsurg floor that had a channel for patients to use for educational material called the Get-Well Network. My friend mentioned that they do not have this educational channel on the NICU floor, and she witnesses many parents overwhelmed with anxiety due to not fully understanding the condition their baby is in. After discussing this issue with her I decided that it would be great to advocate for this educational channel on the NICU floor.

The issue may be difficult to recognize, but most parents on the NICU floor are overwhelmed with their babies being in this condition that learning education on the condition can be challenging. Us nurses and doctors sometimes do not always make it the easiest to understand education on their baby’s condition. A study provided evidence that the parent’s lowest level of satisfaction on a NICU floor was related to providing necessary education during discharge (Salehi, Nouri, Khademolhoseyni, and Ebadi, 2014). Providing necessary education to the parents can decrease the level of their anxiety. Strategies I plan to implement for this issue and how I believe this will be effective in addressing the issue is by incorporating the Get-Well network on the NICU floor. This is a channel that has many informative educational videos that the parents can watch while they are in the room with their babies. The videos will contain information on the baby’s condition, behavioral characteristics to be anticipated, behavioral characteristics that are unanticipated, and how to care for their babies.

One of the most widely and probably under talked about issues is alarm fatigue and the effect it has on patient safety. Alarm fatigue is a real problem and most nurses don’t even realize that they are alarm fatigued. I identified this issue when my facility had a new roll-out of a system called a “tele rapid response”. This would happen over the hospital-wide PA system and would alert if a patient came off cardiac telemetry monitoring. This was put into place when two patient expired within a month of each other due to the nurse not doing hourly rounds on the patient when off telemetry. Unfortunately the patients were found expired during morning shift report. Since this new system has been in place for the last two months, there have been three instances where a real rapid response has been ignored and had to be called more than two or three times over the hospital-wide PA system. Which could result in delay of care and unsafe patient outcomes.

Alarm management is one of the Joint Commission’s National Patient Safety Goals (JC, 2019) because sentinel events have directly been linked to the devices generating these alarms, it’s also estimated that 80-99% of alarms are not actionable. Many of these “false alarms” come from specific patient alarm limits set too low.

A strategy that can be used is defining specific patient alarm limits. By limiting what alarm limits are for a each specific patient, this will decrease the amount of alarms requiring in actionable items. Which will also directly help dampen alarm fatigue which will allow nurses and other healthcare workers to respond to the actionable alarms.
